Formation is a team app for basketball clubs. This policy explains what we collect, why, and the choices players, parents and coaches have.

What we collect

• Account details: name, email and date of birth.
• Team details: role, jersey number, position and class.
• What you add to your team: schedule replies, attendance, chat messages, tasks, workouts, meal orders, outfit votes, stats, film and files.
• Wellness check-ins, from players 18 and older only.
• Basic usage: which weeks you opened the app, so the club can see whether it's being used.

Who can see it

Only approved members of your team. Coaches and staff see the whole team; players see their own records and shared team content. Removed members lose access right away. We do not sell personal information or show ads.

Children under 13

A player under 13 can't be approved until a parent or guardian gives consent through a link we provide. Parents can review or delete their child's information by contacting us.

Wellness check-ins

Check-ins are turned off for players under 18. For adults, an automated tool may suggest that a coach follow up; a person always makes the decision. Check-ins are not medical care and are not monitored around the clock. In an emergency, call 911. For a mental health crisis, call or text 988.

How long we keep it

We keep team content while the team uses Formation and delete an account's information on request, unless the law requires us to keep it.

Your choices

You can ask to see, correct or delete your information. California residents have additional rights under California law.
